To bring on your first visit:
- Check your insurance coverage if you require a medical note and if you do Rosedale Wellness Centre can help coordinate a doctors note
- Arrive 10-15 minutes before your appointment to fill out some paperwork or you can conveniently do it online through our patient portal
- In case of an automobile accident, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number
- Please wear comfortable clothing that allows easy access to your problem areas.
